Origins:   The above-displayed photographs first hit our inbox in November 2006 and purportedly show a man whose head was crushed when a piece of heavy construction equipment rolled or flipped off a flatbed truck and landed atop him. We haven't yet been able to confirm any details of the accident, but the context of the photograph indicates it took place at a shipyard in Singapore.
